The staff at the Student Health and Wellness Center (SHWC) is committed to providing quality health care for the students of the Delaware Valley University campus. In order to accomplish this, we need accurate and up-to-date information including required immunization records. This requirement is for full-time freshmen and transfer students new to DelVal. Please return the completed form to the Student Health and Wellness Center no later than August 1 for the fall semester (January 10 for spring semester).
All full-time students who are an Equine Management Major, plan to take an equine riding skills course, or planning to participate in an equine riding team (Hunt-Seat, Western, or Dressage) are required by the Equine Department to submit an annual physical.
For athletes: If you intend to participate in intercollegiate sports (including Hunt-Seat team and Dressage team) the athletic training department requires that your physical be performed on or after June 1 and all Athletic health forms be returned to Delaware Valley University by August 1. Waive, or Enroll in, the Student Health Plan
Insurance verification information must be done yearly. All full-time (12 or more credits) undergraduate students in the 2017 fall term must enroll or waive the University's annual health insurance. If you do not go online starting June 1, 2017 and verify your insurance plan or enroll in the University plan by the deadline of September 22, 2017, you will be put on registration hold. IMPORTANT: You will be charged a $200 late fee if you register as full-time by July 31, 2017 and fail to either waive the insurance or make approved arrangements to pay the premium by that date, or if you register full-time after August 1, 2017 and fail to either waive the insurance or make approved arrangements to pay for it before your arrival on campus. If you do not respond by September 22, 2017 the charge of $1,741 (or $1,883 for summer bridge students) for the annual insurance will remain on your account and you will be automatically enrolled in the health insurance plan. This charge is non-refundable.
