The staff at the Student Health and Wellness Center (SHWC) is committed to providing quality health care for the students of the Delaware Valley University campus. In order to accomplish this, we need accurate and up-to-date information including immunization records. This requirement is for full-time freshmen and transfer students new to DelVal. Please return the completed form to the Student Health and Wellness Center no later than August 1 for the fall semester (January 10 for spring semester).

 All full-time students who are an Equine Management Major, plan to take an equine riding skills course, or planning to participate in an equine riding team (Hunt-Seat, Western, or Dressage) are required by the Equine Department to have an annual physical. 

For athletes: the athletic training department requires that your physical be performed on or after June 1 and all forms be returned to Delaware Valley University by August 1. Please see the athletic training website for additional  information


Waive, or Enroll in, the Student Health Plan

All full-time students must enroll or waive the University's health insurance. You are required to enroll or waive the Student Health Plan annually while a student at Delaware Valley University. Please go online beginning June 1 for the fall term (beginning December 8 for the spring semester) and verify your insurance plan or enroll in the university plan. If you do not respond, your account will be charged the Student Health Plan fee which is non-refundable. Coverage effective dates are from August 1, 2016 through July 31, 2017.

IMPORTANT: You will need to have your student id number and current health insurance card in order to complete the waiver process.  Please use your 7 digit student id number, including the leading zero. If your record is not found, try and use your id number without the leading zero. If your record still is not found, please contact the Bursar's Office for assistance.
