Athletic Training - Athletic Insurance Program

Student Athletes and Parent(s) and Guardian(s):

Please read and familiarize yourself with this document. This document is for you to keep in your records.

It is expected that each student athlete possess an insurance card and prescription card with the proper information on it. If you do not have an extra card, one can be requested through your insurance carrier or you can photocopy both sides of your card. (Copy of card should be with students at all times).

All student athletes who attend DELAWARE VALLEY COLLEGE must carry health insurance upon entrance to school. This is a College policy, one that the athletic department adheres to implicitly. For this reason an others, it is very important that we have accurate and complete insurance information in reference to the student athlete. This form of insurance will be identified as their primary insurance.

Student Athletes who are cleared for participation by the Athletic Administration and Athletic Training Staff will be eligible to be covered by our athletic insurance policy, which will be identified as secondary insurance. This coverage is purchased by DELAWARE VALLEY COLLEGE. This policy is designed to assist in covering expenses only when the primary coverage does not cover the entire cost of the claim. Furthermore, it only covers injuries incurred while the student athlete is engaged in practice and/or competition of his/her sport. Illness or sickness will not be covered under this secondary policy. With this secondary policy there is a $250 deductible which will be used initially before the student athletes primary policy is contacted.

For a claim to be processed in an efficient and expedient manner. Please follow the steps outlined below:

  1. All injuries must be reported to member of the Athletic Training Staff in a timely fashion. Failure to notify the Athletic Training Staff of an injury could result in the school's sport insurance policy denying your claim. Thus, you and your insurance policy would be held accountable for all medical bills.
  2. A clinical observation will be made by one of the Certified Athletic Trainers and a determination for further medical referral to physician will take place. Any doctor or hospital visits without the athletic trainer’s approval will not be covered by the school’s athletic insurance policy and would fall entirely on the student athlete and their primary insurance.
  3. If further medical attention is warranted (physician, office visit, x-rays, etc.) and the student athlete has exceeded the $250 deductible, then their primary insurance must be used. Also at this time the student athlete must complete a claim form for secondary insurance.
  4. After the primary insurance receives the claim and processes it, you will receive an explanation of benefits (EOB). This document will indicate what service is covered. If there is a balance remaining on the bill, the secondary policy will pick up coverage and pay the remainder of the balance.
  5. The EOB and all itemized bill(s) should be sent to:

  6. Most of the time, the balance will be paid in full, however, there are limits to this policy. In the event that this takes place, you will be financially responsible for the remainder of the balance.

*If you are a member of an HMO, PPO or a POS and have a primary Care Physician, please make the Athletic Training Staff aware of this before we send your son/daughter to our Team Physician.
